How to Request a Duplicate of the Military Parachutist Certificate and Personal Jump Logbook

Have you lost your military parachuting certificate and/or jump logbook? No worries, just submit a request directly to the CAPAR (Parachuting Training Center, formerly SMIPAR) in Pisa and you will receive your duplicate.

Let's now look at the procedure to follow case by case (we have 3 options).

Option 1: Certificate Card and Personal Jump Logbook

1. Application on plain paper specifying (you can download a template by clicking here);
2. Copy of valid identity card and tax code;
3. 3 passport-sized photos, 1 of which authenticated at the municipality of residence;
4. Copy of the loss report to the competent authorities (Carabinieri or Police);
5. Copy of bank transfer receipt:
   • AMOUNT: 28.66€ which includes:
     • € 16.00 stamp duty for every 4 sheets produced (excerpt);
     • € 5.00 cost of certificate card and personal jump logbook;
     • € 7.40 reimbursement for registered mail R/R response:
     • € 0.26 for each sheet produced (excerpt);
   • PAYABLE TO: COMANDO BRIGATA PARACADUTISTI FOLGORE
   • IBAN: IT17 C076 0113 9000 0001 2882 577
   • REASON: Issuance of duplicate certificate card and personal jump logbook of (your name and surname);

Option 2: Certificate Card Only

1. Application on plain paper specifying (you can download a template by clicking here);
2. Copy of valid identity card and tax code;
3. 3 passport-sized photos, 1 of which authenticated at the municipality of residence;
4. Copy of the loss report to the competent authorities (Carabinieri or Police);
5. Copy of bank transfer receipt:
   • AMOUNT: 9.90€ which includes:
     • € 2.50 cost of certificate card;
     • € 7.40 reimbursement for registered mail R/R response:
   • PAYABLE TO: COMANDO BRIGATA PARACADUTISTI FOLGORE
   • IBAN: IT17 C076 0113 9000 0001 2882 577
   • REASON: Issuance of duplicate certificate card of (your name and surname);

Option 3: Personal Jump Logbook Only

1. Application on plain paper specifying (you can download a template by clicking here);
2. Copy of valid identity card and tax code;
3. Copy of bank transfer receipt:
   • AMOUNT: 26.16€ which includes:
     • € 16.00 stamp duty for every 4 sheets produced (excerpt);
     • € 2.50 cost of personal jump logbook;
     • € 7.40 reimbursement for registered mail R/R response:
     • € 0.26 for each sheet produced (excerpt);
   • PAYABLE TO: COMANDO BRIGATA PARACADUTISTI FOLGORE
   • IBAN: IT17 C076 0113 9000 0001 2882 577
   • REASON: Issuance of duplicate personal jump logbook of (your name and surname);
